How the quality index is calculated
Пациент 68 лет. Сепсис. Системная воспалительная реакция. Абсцесс правой доли печени. Сопутствующий диагноз: Внебольничная двусторонняя полисегментарная пневмония. Малый гидроторакс справа. Хронический панкреатит. Хронический холецистит. Сахарный диабет 2 типа. Целевой гликированный гемоглобин 7,0%. Учитывая преобладание общемозговой симптоматики и выраженные когнитивные нарушения, наличие нескольких очагов гнойного воспаления, лейкоцитоз со сдвигом лейкоформулы влево в ОАК, прокальцитонин 2-10, С-РБ 292,5 мг/л нельзя исключить воспалительный характер процесса в головном мозге с вторичным кровоизлиянием. А на самом деле у него тромбоз левой ВСА и глубокий инфаркт в бассейне М1 и А1 слева (в остальных отделах кровообращение компенсировано, кстати, о чем и говорит геморрагическая трансформация инфаркта: есть откуда крови в некроз течь).
Планирование нужно, если превращается в действие — и должно быть отброшено, если начинает служить щитом и оберегом от действия. Планы подготовки нужно отбрасывать как лишний хвост, если мы видим, что они не воплощаются ни во что, кроме продолжения планирования подготовки. Если мы подменяем деяния проработкой планов, то мы однажды начнём кочевать по нетворкам в кредит — и рассказывать там в пиджаках про наш коричневый пояс по осознанности и диплом по голоданию.
Часто спрашивают: а вот это внутримозговая арахноидальная киста или нейроэпителиальная киста с выходом на контур полушария? Или это арахноидальная киста, снаружи вдающаяся вглубь полушария мозга? А правильным ответом на этот вопрос будет: да какая разница? И так и другая может быть внутри мозга, это доказанный факт. И так и другая можен выглядеть, как будто она изнутри мозга растёт наружу и достигает субарахноидального пространства (или наоборот, извне внедряется вглубь). И так и другая не накапливают в стенке контраст. И так и другая, чаще всего, никак не проявляются клинически. И та и другая, если проявляются клинически, требуют, как минимум, како-го то вмешательства (дренирования, фенестрации, некоторые их вообще умудряются иссмекать со стенкой, но это редкость). И та и другая могут быть распознаны, как таковые, только путём гистологического исследования стенки. Так какого же рожна Вы хотите их отдифференцировать по МРТ, если это невозможно, да и не нужно? Поэтому пишем: арахноидальная с внутримозговым компонентом, либо нейроглиальная с выходом на поверхность, либо внутримозговая арахноидальная киста в теменной доле/области справа сомнительного клинического значения.

Энцефалоцеле поперечного синуса - не классическая врождённая черепно-мозговая грыжа (при которой мозговая ткань выходит через костный дефект черепа наружу), а редкая разновидность грыжи мозговой ткани в полость венозного синуса твёрдой мозговой оболочки, при которой височная или затылочная доля мозга вдавливается в просвет поперечного синуса. В литературе её также называют «скрытым энцефалоцеле» (occult encephalocele) или «грыжей мозговой паренхимы в твёрдую мозговую оболочку». У большинства пациентов симптомов нет (то есть, она обнаруживается случайно), но в литературе описана связь с головной болью, головокружением, обмороками, пульсирующим шумом в ушах и даже парциальными эпилептическими приступами. Имеется статистически значимая связь с идиопатической внутричерепной гипертензией: у пациентов с пульсирующим шумом в ушах при наличии такой грыжи вероятность сопутствующей ИВГ примерно в 4 раза выше, чем при её отсутствии. Самое главное - не путать с тромбозом синуса и с арахноидальной грануляцией (в отличие от последней, у грыжи структура негомогенная и не напоминает ЦСЖ).

Ретроградный заброс контраста в печёночные вены (Retrograde reflux of contrast medium into the hepatic veins). Красиво, но не страшно. Чаще всего обусловлено слишком большой скоростью введения контраста через автоматический инжектор, но может быть признаком и сердечной недостаточности и/или лёгочной гипертензии. Кстати, здесь и размеры камер сердца больше верхней границы нормы для аксиальной КТ, и гидроторакс двусторонний, возможно, здесь и не в инжекторе дело.
